Alexis McCray, Ed.S., LPCC, LPC, is approved or qualified to provide clinical supervision within the scope of her licenses and applicable state requirements.
All supervision is conducted online through a secure platform.
States and License Types Served
Clinical supervision may be available for:
New Mexico
Alexis is a New Mexico Licensed Professional Clinical Counselor and holds an Approved Clinical Supervisor Designation from the New Mexico Counseling and Therapy Practice Board.
Supervision is available to qualifying New Mexico Licensed Mental Health Counselors working toward independent licensure.
Before supervision begins, the required New Mexico experience verification plan and EHMH supervision agreement must be completed.
Colorado
Alexis is a Colorado Licensed Professional Counselor and may provide clinical supervision to qualifying Colorado Licensed Professional Counselor Candidates working toward independent licensure.
The supervisee is responsible for confirming that the proposed supervision arrangement and format meet all current Colorado licensure requirements.
Arizona
Alexis is an Arizona Licensed Professional Counselor and may provide clinical supervision to qualifying Arizona Licensed Associate Counselors working toward independent licensure.
Depending on the supervisee’s employment setting, Arizona may require additional documentation or approval for supervision provided by a supervisor who is not employed by the same agency. The supervisee and employer are responsible for confirming and completing any applicable requirements.
Texas Exclusion
Alexis is not approved to supervise Texas LPC Associates.
Elevated Horizons Mental Health does not provide supervision for hours intended to satisfy Texas LPC Associate licensure requirements.
Supervision Is Available Outside EHMH Employment
Supervisees are not required to be employed by Elevated Horizons Mental Health or to have previously worked with EHMH.
Clinical supervision is offered as a separate professional service. Receiving supervision from Alexis does not create employment, contractor, internship, partnership, or agency status with EHMH.
When a supervisee works for another organization, the supervisee is responsible for confirming that the employer permits outside supervision. The employer may be required to participate in or acknowledge the supervision arrangement depending on the state, work setting, and applicable licensing requirements.
EHMH does not assume responsibility for the supervisee’s employment, compensation, client assignments, workplace policies, onsite emergencies, or administrative management.
Supervision Formats
Supervision may be provided in the following formats, subject to state requirements and availability:
Individual supervision: One supervisor and one supervisee
Dyadic supervision: One supervisor and two supervisees
Triadic supervision: One supervisor and three supervisees
Group supervision: One supervisor and multiple qualifying supervisees
Supervisees may participate in more than one format when permitted by their licensing board and supervision plan.
The supervisee is responsible for understanding how many individual, dyadic, triadic, or group hours their state will accept toward licensure.

Supervision is collaborative, but it is also evaluative. The supervisor is responsible for monitoring clinical development, providing feedback, documenting progress, and addressing concerns that may affect client safety or professional competence.
Getting Started
1. Schedule a Consultation
The process begins with a consultation to discuss the supervisee’s license type, state, employment setting, professional goals, supervision requirements, and preferred format.
2. Verify Eligibility
The supervisee must provide proof of their current license or candidate status, applicable board requirements, employment information, and any required supervision forms.
3. Confirm the Supervision Plan
The supervisor and supervisee discuss scheduling, fees, communication expectations, documentation, emergency procedures, evaluation methods, and the responsibilities of each party.
4. Complete the Required Agreements
A written EHMH clinical supervision contract and any required state or employer forms must be completed before supervision begins.
5. Begin Supervision
Supervision hours begin only after all required documents have been signed and the supervision relationship has been formally established.
Supervision hours will not be approved retroactively for services provided before the effective date of the agreement.
Fees
Initial supervision consultation: $50
Individual supervision: $100 per session
Group supervision: $50 per supervisee, per session
Four individual sessions: $350
Two individual and two group sessions: $325
Dyadic and triadic supervision fees depend on the session structure, number of participants, and applicable state requirements. Exact fees will be confirmed before scheduling.
All fees are self-pay. Payment is required according to the terms of the supervision agreement.
All supervision fees are nonrefundable.
Missed sessions, late cancellations, document requests, and other administrative services may result in additional charges as described in the supervision agreement.
Community Service Discount
A 10% discount is available on standard pay-as-you-go supervision sessions for eligible:
Active-duty service members
Military spouses
First responders
Dependents of active-duty service members or first responders
Educators
Current proof of service, employment, or eligible dependent status is required before the discount is applied.
The discount:
Applies only to standard individual, dyadic, triadic, or group session fees
Does not apply retroactively
Does not apply to packages, consultations, administrative fees, late-cancellation fees, document fees, or other charges
Cannot be combined with another discount
Must be renewed when eligibility documentation expires or changes
Records and Document Requests
EHMH maintains supervision contracts, attendance records, supervision notes, evaluations, and applicable supervision logs for at least seven years, or longer when required by law, board rule, contract, or another applicable requirement.
Supervisees should maintain their own copies of all supervision logs, evaluations, board forms, and supporting documentation. EHMH is not responsible for replacing records the supervisee failed to retain.
An administrative fee applies when a supervisee requests copies, verification, completion of replacement forms, extensive record retrieval, or additional documentation outside routine supervision services.
The applicable fee and estimated completion time will be provided before the request is processed.
Clinical Supervision Disclosure Statement
Alexis McCray, Ed.S., is independently licensed as a Licensed Professional Clinical Counselor in New Mexico and as a Licensed Professional Counselor in Colorado and Arizona. She holds an Approved Clinical Supervisor Designation issued by the New Mexico Counseling and Therapy Practice Board.
Clinical supervision is offered only within the scope of the supervisor’s active licenses, qualifications, competence, and applicable state laws and board rules.

A supervisor-supervisee relationship is established only after:
The prospective supervisee has been screened and accepted
Eligibility and license status have been verified
All required state, employer, and EHMH documentation have been completed
The supervisor and supervisee have signed a written clinical supervision contract
Any required supervision plan or board form has been completed
The effective date identified in the agreement has begun
Scheduling a consultation, submitting an inquiry, making a payment, attending an informational meeting, or exchanging documents does not independently establish a supervision relationship.
Acceptance for supervision is not guaranteed. EHMH may decline a proposed supervision arrangement when it falls outside the supervisor’s competence, capacity, jurisdiction, or professional requirements.
The supervisee remains responsible for:
Understanding and following the requirements of their licensing board
Confirming that supervision hours and formats are eligible for licensure credit
Maintaining an active and unrestricted license or candidate credential
Practicing only within their authorized scope
Following employer and agency requirements
Maintaining accurate copies of supervision records and logs
Notifying the supervisor of changes to employment, license status, complaints, investigations, disciplinary actions, or other material circumstances
Submitting licensure applications and supporting documents by applicable deadlines
Licensing boards retain final authority to determine whether supervision hours, supervisors, documentation, and experience satisfy licensure requirements. EHMH cannot guarantee that a board will accept particular hours or approve an application for independent licensure.
Clinical supervision is not psychotherapy, legal representation, employment, or a guarantee of professional advancement or licensure.
