To schedule an appointment, you can contact us by phone, text, or email. After a brief discussion with our receptionist to assess your needs, you’ll complete intake paperwork and provide any necessary documents. Once this is done, our team will work with you to schedule your first session.

Yes, we accept Blue Cross Blue Shield of Texas insurance. For those without insurance, we offer flexible pay-per-session options or subscription plans to make mental health care more accessible. Our receptionist will help you navigate payment options during the scheduling process.

eGift Card Policy
At Elevated Horizons Mental Health, we’re honored to be part of your healing journey—and now, you can share that journey with others through our digital gift cards. Please review the following policy before purchase or redemption:
Terms & Conditions:
eGift cards are valid only for services offered directly by Elevated Horizons Mental Health, PLLC.
Cards cannot be used for insurance co-pays, third-party services, or prescription-related expenses.
eGift cards are non-refundable, non-transferable, and not redeemable for cash.
eGift cards do not expire and may be applied to any eligible service unless otherwise specified at the time of purchase.
Lost, stolen, or damaged cards will not be reissued, so please store your gift email securely.
No cancellation or no-show fees apply when using a gift card to book consultations or services.
Redemption Process:
To use your eGift card, simply contact our office or book your session online. Enter the gift card code at checkout or provide it to our team when confirming your appointment. If the service exceeds the gift card value, the balance will be due at the time of service.

No-Show & Late Cancellation Policy
At Elevated Horizons Mental Health, we value your time and our providers’ commitment to your care. To ensure fairness and efficiency, we enforce the following policy for missed or late-canceled appointments:
What Counts as a Late Cancellation or No-Show?
Late Cancellation: Any appointment canceled within 48 hours of the scheduled time.
No-Show: Failure to attend your appointment or arriving more than 10 minutes late without notice.
Fee & Payment Terms
A $125 fee will be charged for all late cancellations and no-shows.
This fee is non-refundable and must be paid before services can continue.
We may charge the card on file or send an invoice for payment.
Exceptions
We understand that emergencies happen. You will not be charged if the missed appointment is due to:
A documented medical emergency
An act of God (e.g., natural disaster, severe weather)
EHMH needing to reschedule due to provider availability (not the patient’s fault)
We appreciate your understanding and cooperation in respecting our time and resources so that we can continue offering timely care to all who need it.
Evaluation & Letter Policy
Elevated Horizons Mental Health provides fee-based evaluations and letters for specific clinical needs. This includes:
Emotional Support Animal (ESA) Evaluations
Nexus Letters for Veterans
Letters Supporting Gender-Affirming Surgery
Service Overview
These services are typically completed within 1–2 telehealth sessions.
A Release of Information (ROI) may be required to coordinate with other providers.
Clients are encouraged to submit relevant medical or mental health records to support their diagnosis or request.
In some cases, ongoing treatment or establishing care with EHMH may be necessary before a letter can be issued.
Payment & Delivery
Full payment is due prior to letter issuance.
Each letter includes one administrative correction free of charge (e.g., spelling error, formatting).
Letters are valid for up to 12 months from the date of issuance unless otherwise noted.

Student Training, Site Supervision & Licensure Hours Policy
Elevated Horizons Mental Health is proud to support the next generation of mental health professionals. To ensure high-quality training and alignment with our mission, we require the following for all individuals seeking site placement, internship hours, or supervision toward licensure:
Placement Requirements
All placement inquiries must be made at least 6 months in advance of your proposed start date.
A minimum 6-month commitment is required for individuals pursuing hours toward clinical licensure.
EHMH cannot guarantee immediate access to a caseload or waitlist. Please plan accordingly if your program requires a minimum number of client hours within a set timeframe.
A signed Non-Solicitation, Confidentiality, Intellectual Property, and Nondisclosure Agreement is required prior to the start date.
Compensation & Pay Structure
Students are not compensated for their internship or training hours.
Provisionally licensed providers (e.g., LMSWs, LPC-As) set their own rates and are paid under a 60/40 revenue split:
60% to EHMH
40% to the licensee
Payments are issued monthly and only apply to clinical hours rendered (billable sessions only).
Marketing & Outreach
All trainees and provisionally licensed providers are expected to assist in marketing EHMH services in conjunction with their own outreach. Growing your caseload is a shared effort.
Application Materials
To be considered for placement, please submit the following:
Resume or CV
School program requirements and documentation of hour expectations
Written notice from your clinical supervisor or program director
Proof of professional liability insurance
Professional references
Completion of an interview with the EHMH team

Continuity of Care & Departure Expectations
All patients and intellectual property acquired or developed during your time with EHMH remain the sole property of Elevated Horizons Mental Health.
A discharge summary is required for all clients prior to your departure.
You must provide a warm hand-off or transfer of care for every client, including referrals if necessary.
All projects and responsibilities must be transitioned to designated staff members in a professional manner.
Our core expectation: Leave the practice better than you found it.
Policy Compliance
All supervisees and students are expected to fully comply with EHMH’s internal policies, university/program requirements, and any clinical supervision agreements. Continued placement is based on professionalism, communication, adherence to policy, and alignment with EHMH’s mission.
Corporate Trainings & Speaking Engagements Policy
Elevated Horizons Mental Health offers impactful, evidence-based trainings and speaking engagements tailored for corporations, government agencies, nonprofits, and community organizations. Our goal is to promote mental wellness, emotional intelligence, and trauma-informed practices in every environment.
Pricing & Fees
In addition to the base cost of the course or presentation, the following additional fees may apply:
Travel and accommodation expenses
Printed materials or workbooks
Access to digital content, recordings, or presentation slides
Duration of the session (hourly or full-day rates may vary)
Size of the audience, including number of employees or attendees
A signed contract and full payment are required prior to the start of any event.
Expanded Services & Support
We encourage organizations to:
Bundle follow-up sessions for employees who were unable to attend the live session
Explore contract opportunities for long-term wellness support and consultation
Pre-purchase therapy sessions or service credits for employees to use directly through Elevated Horizons Mental Health
Post-Event Evaluation
All attendees will be asked to complete an evaluation form to provide feedback on the training and/or speaker. These evaluations help inform the quality and development of future offerings by EHMH staff and providers.

Insurance Use Policy
Elevated Horizons Mental Health is committed to providing accessible and transparent care. We accept select insurance plans and work with AY Billing, LLC for billing, credentialing, insurance verification, and co-pay confirmation.
Insurance Panel Participation
We participate with insurance carriers at the discretion of the CEO, and insurance panels may change at any time.
Clients are encouraged to contact our administrative team prior to scheduling to verify whether we are currently in-network with their insurance plan.
Important Insurance Considerations
We do not provide education or consultations on insurance benefits.
Please refer to our Superbill Guide for information on submitting claims independently when using out-of-network benefits.Services provided by student interns or for specialized offerings (e.g., ESA evaluations, Nexus letters, gender-affirming surgery letters) are not billable to insurance due to the lack of CPT codes. These services are self-pay only.
Clients are responsible for all fees not covered, denied, or rejected by their insurance carrier.
If insurance fails to reimburse for three consecutive therapy sessions, EHMH reserves the right to pause treatment until payment is received.
Payment & Reimbursement
Insurance payments are not immediate and processing timelines depend on the insurance carrier.
Reimbursement rates for services may vary, and self-pay rates may differ from insurance reimbursement amounts.
EHMH may experience claw backs (insurance overpayment retractions) when reimbursement rates change. These amounts may be billed back to the client when applicable.
Considering Self-Pay Options
If you have difficulty finding in-network providers or your plan does not accept superbills, you may wish to explore opting out of behavioral health coverage and using self-pay options for more flexible care.
