The first step for most Elevated Horizons Mental Health services is a consultation. This allows us to briefly discuss what you need, determine whether the requested service may be appropriate, and explain the next steps.
You do not need to complete extensive intake paperwork before booking your consultation.
Therapy Consultation
Start Individual Therapy
Individual telehealth therapy is available to eligible clients ages 13 and older who are physically located in Texas, New Mexico, Arizona, or Colorado during appointments.
Therapy may address anxiety, trauma, stress, grief, identity, military experiences, relationship patterns, family concerns, and major life transitions.
The consultation provides an opportunity to discuss:
What is bringing you to therapy
Your general goals
Telehealth suitability
Scheduling and availability
Fees and payment expectations
Whether EHMH may be an appropriate fit
Booking a consultation does not guarantee acceptance for ongoing therapy.
Evaluations and Professional Letters
Start with an Evaluation Consultation
A consultation is required before beginning most specialized evaluation and professional-letter services.
Immigration Psychological Evaluations
Trauma-informed evaluations for matters involving:
Asylum
VAWA
U-Visas
T-Visas
Extreme hardship
Nexus Letters for Veterans
Independent clinical evaluations and nexus-letter services for eligible veterans pursuing disability claims involving PTSD or military sexual trauma.
Gender-Affirming Evaluations and Letters
Respectful evaluations and supporting documentation for adults seeking gender-affirming medical care.
Important Evaluation Notice
Payment for a consultation or evaluation does not guarantee:
A diagnosis
A professional letter
A particular clinical opinion
A favorable legal or administrative outcome
Approval by an insurer, landlord, university, physician, attorney, VA reviewer, court, or government agency
All clinical opinions and documentation must be supported by the completed evaluation and applicable professional standards.
Emotional Support Animal Evaluations
Ready to Book an ESA Evaluation?
Eligible renters and college students in Texas, New Mexico, Arizona, and Colorado may schedule an emotional support animal evaluation directly without completing a separate consultation first.
Evaluations may address up to two animals.
Clinical evaluation: $250
ESA letter, when clinically supported: $150
Booking and paying for an evaluation does not guarantee that an ESA letter will be issued. EHMH does not sell automatic or guaranteed letters.
Clinical Supervision
Supervision for Counselors Pursuing Independent Licensure
Clinical supervision is available to eligible counselors pursuing independent licensure in:
New Mexico
Arizona
Colorado
EHMH does not provide supervision for Texas LPC-Associates.
A supervision consultation allows us to review your current credential, employment setting, supervision needs, state requirements, scheduling, and professional goals.
A 10% supervision discount is available to eligible military members, veterans, first responders, and members of their immediate families.
Parent Coaching
Practical Support for Parents and Caregivers
Parent coaching is available for caregivers who want focused assistance with:
Communication
Parenting stress
Family transitions
Boundaries
Behavioral concerns
Changing relationships with children or teenagers
Teen-parent coaching is also available to support young parents as they strengthen their caregiving skills and confidence.
Parent coaching is not psychotherapy and is not a substitute for a child’s individual therapy or emergency mental health care.
Elevated Access Program
Apply for Reduced-Cost Counseling
The Elevated Access Program connects eligible clients with supervised master’s- or doctoral-level Counselors-in-Training.
Sessions range from $0 to $60, depending on financial need, program capacity, and provider availability.
Eligible participants must be physically located in Texas, New Mexico, Arizona, or Colorado during appointments.
An application is required because financial eligibility, clinical appropriateness, and Counselor-in-Training availability must be reviewed before placement.
Professional Training
Training for Organizations and Professional Groups
Elevated Horizons Mental Health offers customized training for:

Training may be provided virtually or in person, depending on the location, requested format, audience, and availability.
A consultation allows us to discuss the training topic, audience, learning objectives, format, timeline, and budget before preparing a proposal.
Counselor-in-Training Opportunities
Practicum and Internship Placement
Eligible master’s- and doctoral-level counseling students may apply for practicum or internship opportunities when their universities permit telesupervision and online accumulation of clinical hours.
A placement consultation is required before acceptance.
The consultation allows EHMH to review:

Practicum or internship dates
Availability
Clinical interests
Professional goals
Fit with EHMH’s training model
Payment for a placement consultation does not guarantee acceptance.
Placement depends on university approval, executed agreements, application deadlines, available supervision capacity, and EHMH’s ability to provide an appropriate training experience.
Records and Good Faith Estimates
Request Copies of Records
Existing or former clients who need copies of their records must submit a written request and complete any required authorization or identity verification.
Electronic copies of records maintained electronically are subject to a $6.50 records-copy fee per completed request. Additional permitted charges may apply to paper copies, postage, requested summaries, or third-party requests.
Request a Good Faith Estimate
Individuals who do not have health insurance or who do not plan to use insurance may request a Good Faith Estimate of the expected charges for a scheduled healthcare service.
A Good Faith Estimate may be requested before scheduling or provided after a qualifying service is scheduled.
Before Booking

EHMH is primarily a telehealth practice.
Clinical clients must be physically located in Texas, New Mexico, Arizona, or Colorado during appointments.
Individual therapy is available to eligible clients ages 13 and older.
EHMH does not provide couples or family therapy.
A consultation does not guarantee acceptance for services.
Evaluations do not guarantee letters, diagnoses, recommendations, or particular outcomes.
Payment may be required when scheduling.
All sales are final and payments are nonrefundable, except when otherwise required by law.
A $125 fee applies to late cancellations and missed appointments, except consultations.
Consultation payments remain final and nonrefundable.
Superbills must be requested before the appointment for which they are needed.
Services provided in New Mexico are subject to applicable Gross Receipts Tax.

What Happens After You Book?
After scheduling, you will receive confirmation and instructions for your consultation.
The booking process will ask only for the information needed to schedule and prepare for the initial appointment. More detailed, service-specific paperwork may be provided after the consultation confirms the appropriate next step.
Acceptance for ongoing services may require:
Completion of required paperwork
Confirmation that the service is appropriate
Payment arrangements
Signed consent or service agreements
Secure submission of requested records
Verification of state, licensure, university, or organizational requirements
A consultation is the beginning of the decision-making process—not a guarantee that services will continue.
