A Clear Clinical Opinion Supported by Evidence
Elevated Horizons Mental Health provides mental health evaluations and Nexus letters for eligible veterans pursuing a Department of Veterans Affairs disability claim. The process considers your mental health history, military experiences, relevant records, and the evidence concerning a possible connection between your condition and military service.
UNDERSTANDING THE SERVICE
What Does a Mental Health Nexus Letter Address?
A mental health Nexus letter presents a licensed clinician’s professional opinion about whether the available evidence supports a connection between a diagnosed mental health condition and military service.
The opinion must be based on clinical findings and supporting evidence—not solely on the outcome the veteran hopes to receive. Depending on the request, the evaluation may examine direct service connection, a condition claimed as secondary to another service-connected disability, or the possible aggravation of a condition.
EHMH’s service may include a clinical interview, review of relevant military and medical records, diagnostic assessment when appropriate, and a written professional opinion when the findings support one.
Mental Health Claims Require Individual Review
AREAS OF FOCUS
EHMH evaluates each request according to the veteran’s history, claimed condition, available evidence, and the specific question the clinical opinion is expected to address.
PTSD and Trauma-Related Conditions
Evaluation of claimed mental health conditions associated with military experiences, including combat, traumatic events, or other service-related stressors. Supporting evidence and diagnostic criteria are reviewed as part of the clinical process.
Military Sexual Trauma
Trauma-informed evaluation for veterans whose claimed mental health condition is connected to military sexual trauma. The process is conducted with care and does not require the veteran to disclose unnecessary graphic detail.
What to Expect
Eligibility Review
EHMH first reviews the requested service, claimed mental health condition, current claim stage, and whether the case falls within the practice’s professional scope.
3. Clinical Evaluation
The evaluation may include a detailed clinical interview, mental health history, diagnostic assessment, and discussion of military and post-service experiences relevant to the claimed condition.
2. Records and Evidence
You will be asked to provide available records relevant to the claimed condition and its possible connection to military service. Additional documentation may be requested.
4. Clinical Findings
Alexis reviews the complete information and determines whether the evidence supports a professional opinion. A Nexus letter is prepared only when the clinical findings support one.
Preparing for Your Evaluation
The records needed will depend on the type and status of your claim. EHMH will identify what is necessary during the eligibility review.
Use a normal bullet list:
DD Form 214 or other service documentation
Relevant military or service treatment records
Available VA and civilian mental health records
Prior diagnoses, evaluations, or disability decisions
Current VA rating decision or denial letter, when applicable
Relevant lay or supporting statements, when available
Information about the claimed condition and requested connection
Closing sentence:
Do not delay contacting EHMH solely because you do not have every document. However, the evaluation cannot be completed until the records necessary for a responsible clinical opinion are available.
A Nexus Letter Is Not a Guaranteed Outcome
Payment covers the clinician’s time for eligibility review, record review, evaluation, professional analysis, and applicable documentation. It does not purchase a favorable opinion.
EHMH cannot guarantee that an evaluation will result in a Nexus letter or that the Department of Veterans Affairs will approve a claim, assign a particular disability rating, or accept a clinical opinion. The VA makes all final determinations concerning service connection and disability benefits.
