Individual Therapy
Online Therapy for Teens and Adults
Individual therapy is available to eligible clients ages 13 and older who are physically located in Texas, New Mexico, Arizona, or Colorado at the time of service.
20-minute session: $95
30-minute session: $125
45-minute session: $175
60-minute session: $225
The appropriate session length depends on the client’s clinical needs, treatment goals, and provider availability.
New Mexico services are subject to applicable New Mexico Gross Receipts Tax.
Elevated Access Program
Reduced-Cost Counseling
Eligible participants may receive telehealth counseling from a supervised master’s- or doctoral-level Counselor-in-Training.
Session fee: $0–$60
Sliding-scale and pro bono availability varies
Placement is based on eligibility, clinical appropriateness, financial need, and provider availability
Submitting an application does not guarantee acceptance, immediate placement, or a particular session fee.

Immigration Psychological Evaluations
Evaluations for Immigration Matters
Immigration psychological evaluations may support legal matters involving asylum, VAWA, U-Visas, T-Visas, and extreme hardship.
Initial consultation: $200
Evaluation and written report: $1,400–$3,000
Case preparation or professional consultation: $200 per hour
Half-day testimony: $900
Full-day testimony: $1,600
The final evaluation fee depends on the type and complexity of the case, available records, assessment needs, number of interviews, requested timeline, and anticipated report requirements.

Emotional Support Animal Evaluations
ESA Evaluations for Housing Accommodations
Emotional support animal evaluations are available to eligible renters and college students in Texas, New Mexico, Arizona, and Colorado.
Clinical evaluation: $250
ESA letter, when clinically supported: $150
Evaluations may address up to two animals
Payment for an evaluation does not guarantee that a letter will be issued. Documentation is provided only when supported by the clinical evaluation and applicable professional standards.

Nexus Letters for Veterans
PTSD and Military Sexual Trauma Claims
Nexus-letter services may include clinical interviews, assessment measures, review of available military and medical records, diagnostic considerations, and preparation of an independent clinical opinion.

Clinical Supervision
Supervision for Eligible Counselors
Clinical supervision is available to qualifying counselors pursuing independent licensure in New Mexico, Arizona, and Colorado.
EHMH does not provide supervision for Texas LPC-Associates.
Supervision consultation: $50
Individual supervision: $100 per session
Group supervision: $50 per session
Four individual sessions: $350
Two individual and two group sessions: $325
Eligibility, employment setting, licensure status, supervision requirements, and the proposed arrangement must be reviewed before a supervision agreement begins.
Packages must be used according to the terms stated in the supervision agreement.
Military and First-Responder Discount
A 10% clinical-supervision discount is available to eligible military members, veterans, first responders, and members of their immediate families.
Discounted fees are:
Supervision consultation: $45
Individual supervision: $90 per session

Two individual and two group sessions: $292.50
Eligibility may require verification. The discount applies only to clinical-supervision services and cannot be combined with another discount or promotion.

Parent Coaching
Practical Support for Parents and Caregivers
Parent coaching: $125 for 45 minutes
Teen-parent coaching: $75 for 45 minutes
Parent coaching is not psychotherapy and is not a replacement for a child’s individual mental health treatment or emergency care.

Counselor-in-Training Placement Consultations
Practicum and Internship Opportunities
Placement consultation: $50
Clinical supervision during an accepted EHMH placement is provided without a separate supervision fee
Payment for a consultation does not guarantee placement.
Acceptance depends on fit, university requirements, agreement deadlines, available supervision capacity, and EHMH’s ability to provide an appropriate training experience.

Professional Training
Training for Organizations and Professional Groups
Professional training fees are customized according to:
Length of the presentation
Number of participants
Virtual or in-person delivery

A written proposal will identify the scope, fee, payment schedule, and deliverables before the training is confirmed.

Records and Documentation Fees
Requests for Copies of Client Records
Copies of client records are not provided automatically. Requests must be submitted in writing and include any authorization or identity-verification information required by EHMH.
Electronic copy of records maintained electronically: $6.50 per completed request
Paper copies: Reasonable, cost-based copying and supply charges permitted by applicable law
Mailing: Actual postage and applicable delivery expenses
Record summaries or explanations: Fee quoted in advance when separately requested and agreed upon
Subpoenas, attorney requests, insurance requests, disability documentation, and other third-party requests: Fees assessed according to the request and applicable law
The records-copy fee covers only allowable costs associated with creating and delivering the requested copy. EHMH does not charge clients for time spent searching for or retrieving records.
An estimate of applicable charges will be provided before the records are prepared. Payment may be required in advance when permitted by law.
EHMH will not deny a client access to records solely because the client has an unpaid balance for clinical services.
New Mexico records-copy fees are subject to applicable New Mexico Gross Receipts Tax when required.
Payment and Financial Policies
Payment Policy
Payment is accepted through:

Cash and checks are not accepted unless EHMH provides written authorization for a particular transaction.
Payment must be completed according to the terms provided for the selected service. Depending on the service, payment may be required when scheduling, before the appointment, through an installment agreement, or before a final document is released.
Evaluations, professional letters, reports, testimony, record review, training, supervision packages, consultations, and other reserved professional services may require advance payment.
Clients are responsible for ensuring that payments and invoices are completed by the stated deadline. Failure to complete payment may delay scheduling, appointments, reports, letters, or other deliverables.
Card or invoice payment may be used for records-copy charges. When permitted by law, payment may be required before copies are prepared or released.

Fees for Services Provided in New Mexico
Fees charged directly for services provided in New Mexico are subject to applicable New Mexico Gross Receipts Tax.
The GRT rate may vary based on the service and the appropriate reporting location. When GRT applies, it will be added to the listed service fee and displayed before payment is completed.
A fee shown on the website without GRT should not be interpreted as the final tax-inclusive amount.

Refunds are not provided because a client:
Changes their mind
Cancels a service
Misses an appointment
Does not complete the requested process

Does not receive a requested diagnosis, recommendation, letter, or professional opinion
Does not receive a desired outcome from an insurer, landlord, university, physician, attorney, VA reviewer, court, licensing board, employer, or government agency
Records-copy, postage, document-preparation, and other administrative-service fees are nonrefundable after the requested work has begun.
This policy does not eliminate any correction, refund, dispute, or other remedy required by applicable law. Confirmed duplicate charges and billing errors will be reviewed and corrected.
Cancellation and No-Show Policy
EHMH requires at least 48 hours’ notice to cancel or reschedule an appointment.
A $125 fee applies to late cancellations and missed appointments for all appointment types except consultations.
Consultations are excluded from the $125 late-cancellation and no-show fee. However, consultation payments are final and nonrefundable if the consultation is cancelled, missed, or not rescheduled according to the applicable consultation terms.
Insurance does not pay late-cancellation or missed-appointment fees. These fees are the client’s responsibility and must be resolved before another appointment or service is scheduled.
Separate cancellation terms may apply to training, testimony, reserved group services, or other project-based work when stated in the applicable agreement.
superbills
Superbills Must Be Requested in Advance
Superbills are not issued automatically.
A client who wants a superbill must request it before the appointment for which the superbill is needed. Requests made after an appointment may not be accommodated retroactively.
A superbill is documentation that a client may submit to an insurance company for possible out-of-network reimbursement. It is not an insurance claim, promise of coverage, or guarantee of reimbursement.
Clients are responsible for:
Confirming whether their plan includes out-of-network benefits
Determining whether the service is eligible for reimbursement
Requesting the superbill before the applicable appointment
Submitting the superbill to the insurance company
Responding to requests from the insurance company
Paying EHMH regardless of whether reimbursement is approved
EHMH cannot guarantee that an insurer will accept a superbill or reimburse any portion of the fee.
Good Faith Estimate Notice
Your Right to an Estimate of Expected Charges
Under the federal No Surprises Act, individuals who do not have health insurance or who choose not to use insurance to pay for their care are generally entitled to receive a Good Faith Estimate of the expected cost of scheduled healthcare services.
You may request a Good Faith Estimate before scheduling a service. When applicable, EHMH will also provide one after a service is scheduled within the timeframes required by federal law:
For services scheduled 3–9 business days in advance, the estimate is generally provided within one business day.
For services scheduled at least 10 business days in advance, the estimate is generally provided within three business days.
An estimate requested before scheduling is generally provided within three business days.
A Good Faith Estimate reflects the services reasonably expected at the time it is prepared. It is not a contract and does not require you to receive services from EHMH.
The estimate may need to be updated if your treatment needs, frequency of appointments, requested services, evaluation scope, or anticipated length of care changes.
If a provider’s final billed charges are at least $400 more than that provider’s Good Faith Estimate, you may be eligible to use the federal patient-provider dispute-resolution process.
A dispute generally must be started within 120 calendar days of receiving the initial bill. Keep copies of your Good Faith Estimate and all related bills.

Where are services available?
Therapy and clinical services are available through telehealth to eligible clients physically located in:
Texas
New Mexico
Arizona
Colorado
A client’s physical location—not only their permanent residence—matters. Clients must be located in a state where the treating provider is authorized to practice at the time of the appointment.
Are services offered in person?
EHMH is an online practice.
Therapy, coaching, consultations, evaluations, professional training, and eligible clinical-supervision services are primarily provided through secure video conferencing.
Some evaluation or professional services may require additional coordination depending on their purpose and applicable requirements.
What ages does EHMH serve?
Individual therapy and Elevated Access Program services are available to eligible clients ages 13 and older.
Some specialized services have different eligibility requirements. Gender-affirming evaluations and letters are available to adults ages 18 and older.
Do you provide couples or family therapy?
No. EHMH currently provides individual therapy and does not offer couples or family therapy. We may have a Counselor-in-Training that could offer couples and family therapy, whoever, that would depend on the provider.
Parent coaching is available for caregivers who want practical assistance with communication, parenting stress, boundaries, family transitions, or related concerns.
Who provides therapy?
Individual therapy may be provided by Alexis McCray, Ed.S., LPC.
Reduced-cost counseling through the Elevated Access Program is provided by eligible master’s- or doctoral-level Counselors-in-Training under clinical supervision.
Clients are informed about who will provide their services before treatment begins.
Is a Counselor-in-Training already a licensed counselor?
Not necessarily.
Counselors-in-Training are graduate-level counseling students completing supervised clinical education. They provide services within the requirements of their academic program and under clinical supervision.
Counselors-in-Training are not presented as independently licensed counselors.
Does applying for the Elevated Access Program guarantee free counseling?
No.
Fees range from $0 to $60 based on financial need, provider availability, program capacity, and available funding.
Submitting an application does not guarantee acceptance, immediate placement, or a specific session fee.
How does telehealth work?
Appointments are conducted using secure video conferencing.
Before the first session, clients receive instructions for accessing the appointment and completing required paperwork. Clients should attend from a private location with a reliable internet connection.
The provider may verify the client’s physical location and emergency contact information during each appointment.
Is telehealth appropriate for everyone?
No. Telehealth suitability is considered individually.
Online services may not be appropriate when a person requires:
Immediate crisis intervention
A higher level of care
Intensive monitoring
An in-person assessment
Services that cannot be delivered safely through telehealth
EHMH may recommend another provider or level of care when telehealth is not clinically appropriate.
Do you accept insurance?
Payment expectations and any applicable insurance-billing arrangements are confirmed before services begin.
Coverage varies by plan, provider, state, and service. Specialized evaluations, professional letters, coaching, supervision, consultations, training, late-cancellation fees, no-show fees, record-copy fees, and administrative services may not qualify for insurance coverage.
Clients who plan to seek out-of-network reimbursement must request a superbill before the appointment for which it is needed.
Are superbills issued automatically?
No.
A superbill must be requested before the appointment for which it is needed. Requests made after an appointment may not be accommodated retroactively.
A superbill does not guarantee that an insurer will accept the documentation or reimburse any portion of the service fee.
Can EHMH guarantee that insurance will reimburse me?
No.
Coverage and reimbursement decisions are made by the insurance company. Clients remain responsible for paying EHMH regardless of whether an insurer approves reimbursement.
Are evaluation letters guaranteed?
No.
EHMH does not sell guaranteed letters, diagnoses, recommendations, or favorable professional opinions.
Evaluations are completed independently. Any documentation must be supported by the information collected, applicable clinical standards, and the provider’s professional judgment.
Does an evaluation guarantee approval?
No.
EHMH cannot guarantee decisions made by landlords, universities, physicians, attorneys, insurers, VA reviewers, courts, licensing boards, employers, or government agencies.
The provider’s role is to conduct an appropriate evaluation and offer an independent clinical opinion—not promise a particular outcome.
Does EHMH provide legal advice?
No.
EHMH provides mental health services, clinical evaluations, and professional opinions within the provider’s scope of practice.
Clients with immigration, disability, housing, employment, or other legal questions should consult a qualified attorney.
Is a consultation the same as therapy?
No.
A consultation is used to gather preliminary information, discuss the requested service, determine possible fit, and explain potential next steps.
A consultation does not automatically establish a therapist-client relationship or guarantee acceptance for services.
Can clinical supervision be provided in every state?
No.
EHMH provides eligible licensure supervision in:

Arizona
Colorado
EHMH does not provide supervision for Texas LPC-Associates.
Eligibility must be reviewed before supervision begins because each state has different requirements for supervisors, supervisees, clinical settings, documentation, and acceptable supervision formats.
Who qualifies for the supervision discount?
Eligible military members, veterans, first responders, and members of their immediate families may receive a 10% discount on clinical-supervision services.

Can counseling students apply from outside the four client-service states?
Yes.
Eligible master’s- and doctoral-level counseling students may apply from other locations when their university permits telesupervision and online accumulation of clinical hours.
Clients served through EHMH must be physically located in Texas, New Mexico, Arizona, or Colorado.
Placement depends on university approval, an executed agreement, available supervision capacity, and fit with the practice.
What is the cancellation policy?

Paper copies, postage, requested summaries, and third-party or legal requests may involve additional charges permitted by applicable law. EHMH will provide the expected charge before preparing the records.
The records-copy fee is separate from charges for:

How quickly are evaluations and letters completed?

A projected timeline is discussed before the service begins. A timeline cannot be guaranteed when required information, records, interviews, authorizations, or payments remain outstanding.
What if I am experiencing a mental health emergency?
Elevated Horizons Mental Health is not an emergency or crisis-response service. Messages and online forms are not monitored continuously.
